By Tara Sonenshine (former undersecretary of state) Advocates for “death with dignity” are hopeful that the nation’s capital will be the next jurisdiction where patients facing painful end of life decisions will have the choice to bring their suffering to an end and legally end their lives.

Americans are living longer these days, and with the growth of assisted living, hospice benefits, palliative care units, as well as life-extending health care measures, many have many more options for the last years of life.

At the same time, a quiet movement is growing within various states to address the issue of how people die, and the degree to which a person with a terminal diagnosis is permitted to exercise autonomy and self-determination with respect to the means and timing of their death, as well as the legality/morality of others assisting in hastening death, be it physicians or loved ones.

But rather than have a “debate” over the willingness of all physicians to help those with terminal illnesses hasten death, we need to see the issue as one of empowering mentally able patients with only a six-month likely life span by adding another category of choice, as many states are doing.

While we have learned from the debates within the U.S. states, Canada raise necessary questions around sensitive issues of personal freedom and independence, control, quality of life at the end of life, the cost of prolonging life, dignity in decision-making, and how to instruct others to carry out one’s wishes.

Having all options available for independent decision-making about the end of life comforts patients and leads to wise choices and freedom. Absent choice, the only options available for many are to refuse food and water, or move to one of the six states that allow death with dignity ? not easy roads to travel at the end of life.

‘존엄사’ 옹호자들은 목숨을 끊는 고통스러운 결정에 직면한 환자들이 이 나라의 수도가 자기네 고통을 종결하고 합법적으로 삶을 끝내는 선택권을 행사하는 다음 지자체가 되기를 희망한다.

현재 미국인은 수명이 연장되고 생명을 연장시키는 의료 처치는 물론 노인이나 환자 지원, 각종 호스피스 혜택, 고통 완화 의료시설 확장으로 많은 사람들이 삶의 말기 몇 년에 대한 선택의 폭이 넓어졌다.

그와 동시에 어떻게 죽을 것인가 하는 문제의 해결을 위한 조용한 운동이 여러 주에서 확산되고 있으며, 말기 진단을 받은 사람이 의사든 혹은 사랑하는 사람들이든 죽음의 재촉을 돕는 다른 사람들의 합법성과 도덕성은 물론 자신의 죽는 방법과 시기와 관련하여 행사하도록 허용되는 자주성과 자결권의 정도 또한 높아지고 있다.

말기 환자들의 사망 촉진을 지지하는 모든 의사들의 의향에 대한 ‘토론’을 벌이기보다 우리는 이 문제를, 다른 많은 주에서 하고 있는 것처럼 선택의 또 다른 범주를 추가함으로써 수명이 불과 6개월만 남은 정신적으로 능력이 있는 사람들에게 자율권을 주는 문제로 볼 필요가 있다.

우리가 미국 일부 주에서 벌어진 토론에서 배우는 동안 캐나다는 개인의 자유, 독립성, 통제, 생명 말기의 삶의 질, 생명 연장 비용, 결정 과정의 품위 및 자신의 소원 실현을 위해 타인들에게 당부하는 방법과 관련된 여러 가지 민감한 문제를 둘러싼 필요한 의문을 제기한다.

생명의 종말에 관한 독립적인 결정을 위한 모든 선택 방안을 확보하는 것은 환자들에게 위안이 되며 현명한 선택과 자유로 유도한다. 선택 방안이 없을 경우 많은 사람들에게 유일한 대안은 음식과 물을 거부하거나 품위 있는 죽음을 허용하는 6개 주 가운데 한 곳으로 이동하는 것인데, 이는 생명의 말기에 쉽지 않은 여로이다.
